Who is eligible for care?
To qualify for services from Healer of Hope:
- The patient must be essentially homebound, unable to leave home without assistance, or at least be bedridden but needs skilled treatment.
- The patient needs intermittent care from professionals.
Healer of Hope does not discriminate against any person on the basis of race, color, national origin, disability, or age in admission, treatment, or employment.
Who pays for home health care?
In most cases, home health care services are covered by Medicare, Public Aid, and private insurance providers. Our staff will evaluate your benefits and answer any questions regarding the first home visit.
How do I receive home health services?
Your physician must indicate that home care is necessary in the treatment plan. Our medical director will work with your doctor to help develop the right treatment plan.
When is home care appropriate?
- Care for elderly family members with physical disabilities or those recovering from surgery.
- Stroke rehabilitation therapy at home.
- Wound care that doesn’t require hospitalization.
- Rehabilitation therapy at home.
